For diagnostic purposes, the BNP result should be used in conjunction with other clinical observations. Additional information may be required for diagnosis. In patients with dyspnea, proBNP is highly sensitive for the detection of acute CHF. In addition, a proBNP level of <300 pg/mL effectively rules out acute CHF, with nearly 99% negative predictive value. Please note that marked elevations in proBNP levels may be observed in states other than left ventricular congestive failure, including: acute coronary syndromes, right heart strain/failure (including pulmonary embolism and cor pulmonale), critical illness, renal failure, as well as with advancing age. Falsely low proBNP in CHF patients may be observed with obesity.
