Specimen Requirements

1. Collect fresh specimen and send to lab immediately. Lab needs to add CytoRich as a fixative.

2. Label specimen container or slides with patient’s name (first and last), date and actual time of collection, diagnosis, physician’s name, type of specimen/source, and hospital identification number and department.

Note: 

1. Print Cytology order from Epic and send with specimen. 

2. Include pertinent clinical information - previous cancer, drugs, radiation therapy, immune status, personal history (cigarettes and asbestos exposure, etc.), and forward it with specimen.

3. Causes for rejection:

A. No request form

B. Name on request form does not match name on specimen

C. Unlabeled specimen

D. No doctor’s name given

E. Air-dried artifact
